Pembrolizumab Plus Berahyaluronidase Alfa-pmph Approved for Subcutaneous Administration Across Solid Tumor Indications

    Expert Perspectives · Lyn Brook · 2025-09-29

The FDA has approved pembrolizumab and berahyaluronidase alfa‑pmph (Keytruda Qlex™, Merck) as a subcutaneous injection for adult and pediatric patients ≥12 years with solid tumors for which the intravenous (IV) formulation of pembrolizumab is approved.

    Why This Approval Matters

This approval makes pembrolizumab available through subcutaneous delivery, expanding treatment flexibility while maintaining pharmacokinetic comparability and clinical outcomes with the traditional IV formulation. The subcutaneous option may improve convenience for both patients and clinicians by reducing infusion time and resource use while preserving efficacy and safety.

    Breaking Down the MK-3475A-D77 Trial

Efficacy and pharmacokinetic comparability were evaluated in the MK‑3475A‑D77 trial (NCT05722015), a randomized, multicenter, open-label, active‑controlled study in patients with treatment‑naive metastatic non–small cell lung cancer (NSCLC) without EGFR, ALK, or ROS1 genomic tumor aberrations.

A total of 377 patients were randomized 2:1 to receive either subcutaneous pembrolizumab and berahyaluronidase alfa‑pmph every six weeks with platinum‑doublet chemotherapy or intravenous pembrolizumab every six weeks with platinum‑doublet chemotherapy.

The trial’s dual primary goals were pharmacokinetic (PK) comparability end points: Cycle 1 AUC0‑6 weeks and Cycle 3 steady‑state Ctrough. Both PK endpoints met the predefined acceptance margins, with the geometric mean ratio lower bounds exceeding 0.8, confirming comparability.

Descriptive efficacy end points showed similar outcomes between the arms. Confirmed objective response rate (ORR) was 45% in the subcutaneous arm compared with 42% in the IV arm. No notable differences were observed in progression‑free survival (PFS) or overall survival (OS).

    Managing Adverse Events

The safety profile of pembrolizumab and berahyaluronidase alfa‑pmph was consistent with that of IV pembrolizumab. The prescribing information includes warnings and precautions for immune‑mediated adverse reactions, hypersensitivity and administration‑related reactions, complications of allogeneic hematopoietic stem cell transplantation, and embryo‑fetal toxicity.

    Dosing and Administration

The recommended dosage of pembrolizumab and berahyaluronidase alfa‑pmph is either:

    • 395 mg pembrolizumab + 4,800 units berahyaluronidase alfa‑pmph every three weeks
    • 790 mg pembrolizumab + 9,600 units berahyaluronidase alfa‑pmph every six weeks

Treatment should be administered until disease progression or unacceptable toxicity, or as indicated in the prescribing information.

    What’s New as of September 19, 2025

    • Subcutaneous pembrolizumab plus berahyaluronidase alfa‑pmph approved for adult and pediatric patients ≥12 years across all solid tumor indications currently approved for IV pembrolizumab
    • Provides a new route of administration designed for greater convenience, reduced infusion times, and streamlined clinic logistics
    • Supported by PK comparability data from Study MK‑3475A‑D77, showing equivalent exposure and similar ORR, PFS, and OS versus IV formulation

    Previously Approved Indication

    • Pembrolizumab available only as an intravenous infusion for approved solid tumor indications
    • Longer infusion times (typically 30 minutes) requiring full IV administration
    • Established clinical efficacy and safety profile across multiple tumor types
