Zongertinib Granted Accelerated Approval for HER2 TKD–Mutated Non-Squamous NSCLC

The FDA has granted accelerated approval to zongertinib (Hernexeos®, Boehringer Ingelheim Pharmaceuticals, Inc.), a kinase inhibitor, for adults with unresectable or metastatic non-squamous non–small cell lung cancer (NSCLC) whose tumors harbor HER2 tyrosine kinase domain (TKD) activating mutations, as detected by an FDA-approved test, and who have received prior systemic therapy. In addition, the FDA approved the Oncomine Dx Target Test (Life Technologies Corporation) as a companion diagnostic to aid in detecting HER2 TKD activating mutations in patients with non-squamous NSCLC who may be eligible for treatment with zongertinib.
Why This Approval Matters
This accelerated approval provides a targeted treatment option for patients with HER2 TKD–mutated non-squamous NSCLC after prior systemic therapy, a population with limited effective options. The concurrent approval of a companion diagnostic facilitates appropriate patient selection by reliably identifying tumors with HER2 (ERBB2) TKD activating mutations.
Breaking Down the Beamion LUNG-1 Trial
Efficacy was evaluated in Beamion LUNG-1 (NCT04886804), an open-label, multicenter, multi-cohort trial enrolling patients with unresectable or metastatic, non-squamous NSCLC with HER2 TKD activating mutations who had received prior systemic therapy and were treated with zongertinib. The primary efficacy end points were objective response rate (ORR) and duration of response (DOR), determined by Blinded Independent Central Review (BICR) per RECIST v1.1.
In a cohort of 71 patients previously treated with platinum chemotherapy who had not been previously treated with a HER2-targeted TKI and antibody-drug conjugate (ADC), ORR reached 75%. Amongst responders, 58% maintained a DOR of at least six months. In a separate group of 34 patients who had received both platinum chemotherapy and a HER2-targeted ADC, the ORR was 44%, and 27% had a DOR of six months or longer.
Managing Adverse Events
The prescribing information for zongertinib includes warnings and precautions for hepatotoxicity, left ventricular dysfunction, interstitial lung disease/pneumonitis, and embryo-fetal toxicity. Clinicians should monitor for hepatic laboratory abnormalities, assess cardiac function as clinically indicated, evaluate new or worsening pulmonary symptoms promptly, and implement appropriate pregnancy prevention and counseling measures.
Clinical Implications and Companion Diagnostic
The availability of zongertinib, together with the FDA-approved Oncomine Dx Target Test, supports a precision-medicine approach in advanced non-squamous NSCLC with HER2 TKD activating mutations. The observed objective responses, including in patients previously treated with platinum-based chemotherapy and those with prior exposure to a HER2-targeted ADC, underscore the activity of zongertinib across clinically relevant subgroups.
Dosing and Administration
The recommended zongertinib dosage is based on body weight. For patients weighing less than 90 kg, the recommended dose is 120 mg orally, once daily. For patients weighing 90 kg or more, the recommended dose is 180 mg orally, once daily. Zongertinib may be taken with or without food and should be continued until disease progression or unacceptable toxicity.
